What are the responsibilities and job description for the Case Manager position at Community Health Center of Lubbock?
Community Health Center of Lubbock (CHCL) is seeking a dedicated and compassionate Case Manager to support our mission in improving community health outcomes under the Substance Abuse and Mental Health Services Administration (SAMHSA) grant and other health center programs. The Case Manager will provide comprehensive case management services, facilitate access to health resources, and engage the community through targeted outreach events such as health fairs and educational workshops. The ideal candidate must be a Licensed Baccalaureate Social Worker (LBSW), hold a Community Health Worker (CHW) certification, and possess a strong understanding of healthcare systems and social services in Texas.
Key responsibilities include, but is not limited to:
- Collaborate with other members of treatment team to provide case management to patients with mental health/substance use disorders including coordination with specialty providers and treatment facilities as needed;
- Provide services with a strong recovery orientation, cultural sensitivity, and awareness of the unique needs of the patient population;
- Identify and link patients with community resources to facilitate referrals and respond to social service needs;
- Provide relevant self-management support for patients with mental health/substance use disorders as identified by treatment team;
- Act as a point of contact for patients, families, community resource agencies and treatment facilities in need of information or with concerns;
- Assist patients with the process of determining eligibility, obtaining and maintaining free pharmaceuticals through the Drug Assistance Program;
- Assist with data collection and generation of patient registry reports; and
- Plan and participate in community outreach events such as health fairs, educational workshops, and public awareness campaigns.
Education: A minimum of an associates degree in social work, public health, mental health or related field required. Bachelors degree in social work, psychology or related field preferred.
Licensure: Licensed Baccalaureate Social Worker (LBSW)
Certification: Basic Life Support (CPR) certification; Community Health Worker (CHW) certification
Experience: Minimum of four (4) years progressively responsible healthcare-related experience; additional related education may exchange for required experience on a year-for-year basis; Prefer experience working with individuals with mental health/substance use disorders as well as experience as part of a treatment team approach to care.
Language: English; bilingual (English/Spanish) preferred.
Travel: Some; valid Texas drivers license and current auto insurance required; must have reliable and safe transportation.
Hours of Work: 8am-5pm M-F with some weekend/evening work.
