Job Description Summary

Job Description:

The Center for Key Populations (CKP) Medical Provider provides comprehensive, high-quality primary and specialty care to individuals, families, and groups of Community Health Center clients according to established professional medical and nursing standards. This position works collaboratively with physicians and other members of the healthcare team to meet the preventive, acute, chronic, and health maintenance needs of clients, and collaborates with other community healthcare and human service agencies and clinical/medical specialists as needed.

The CKP Medical Provider provides care in a variety of settings, including working in mobile health units, homeless shelters, and mental health and substance use facilities, as well as other similar venues.

Role And Responsibilities

  • Receive strong clinical support services and work in an innovative practice environment utilizing Advanced Access scheduling
  • Performs comprehensive health histories and physical assessments developing appropriate treatment plans
  • Assesses, diagnosis and manages acute and chronic health problems using the models of planned care and the chronic care model
  • Provides continuity of care to clients by establishing on ongoing primary care/client relationship with individual and families.

Clinical

  • Provides ongoing high-quality primary care to individuals/patients/families
  • Performs all clinical functions within the scope of licensure competently
  • Provides outpatient and preventative medical clinical care consistent with evidence-based best practices in non-traditional healthcare settings like mobile health vans, shelters, food banks, migrant farms and in home, including all CHCI sites
  • Maintains and documents the patient-centered medical record accurately after each client visit and monitor health maintenance requirements of the patient panel according to CHCI policy
  • Participates in an on-call coverage system providing 24-hour care for patients as necessary
  • Works collaboratively with members of the CHCI interdisciplinary team providing medical support and consultation as necessary
  • Participates in other health care activities as directed, e.g. school-based clinic consultant, patient education
  • Actively promotes the care and services of CKP to internal and external healthcare team members.
  • Has expertise in and/or be willing to gain expertise in services that care for vulnerable populations that comprise the Center for Key Populations (CKP), including:
    • HIV and viral hepatitis prevention and treatment
    • STI prevention and treatment, LGBTQIA health and gender-affirming care including hormone therapy
    • substance use disorder treatment including prescribing medication for substance use disorders such as buprenorphine and naltrexone,
    • those experiencing or at risk of being homeless
    • recently incarcerated individuals
  • Provides expertise and knowledge, particularly in CKP services to design curriculums, precept colleagues and students, and present information
Administrative

  • Works collaboratively within an inter-disciplinary team including members of the medical staff, behavioral health staff, and other CHC disciplines or sites.
  • Collaborates with office manager and clerical/clinical staff to promote the smooth functioning of the medical component in meeting patients’ health care needs
  • Participates with CHCI management in developing and achieving optimal clinical performance and organizational goals for productivity and utilization
  • Actively participates in learning modalities such as Weitzman’s Project ECHO and other training opportunities in key populations' clinical competencies
  • Maintains board certification/licensure
  • Attends continuing education programs
  • Maintains involvement in professional organizations as necessary
  • Collaborates with other community health care and human service agencies and clinical/medical specialists as needed.
  • Attains hospital privileges as necessary according to local, site-specific standards and requirements.

Qualifications

Required Education and Skills:

  • Education:
    • MD/DO from an approved medical/osteopathy school. Graduate of an accredited residency program in family practice or internal medicine. Board certified within 24 months of commencing employment at Community Health Center.
OR

  • Graduate of an accredited Master of Science in Nursing program with a specialty as a family or adult primary care nurse practitioner. Maintains board certification as an FNP or ANP. Maintains appropriate board certification, licensure, and professional organizations.

OR

  • Graduate of an accredited Master program in the study of physician assistant. Maintains appropriate board certification, licensure, and professional organizations.
  • Minimum of one year prior experience as an advanced practice registered nurse
  • As a condition of employment, CHC requires current American Red Cross CPR for the Professional Rescuer and AED (CPR/FPR/AED) certification. The only acceptable alternative is the current American Heart Association BLS/AED for Healthcare Providers certification.
  • Excellent written and verbal communication skills to communicate effectively with clients, staff, leadership, and our external partners.
  • Required to develop bilingual capacity as dictated by needs of the site.
  • Interest in working with patients who experience barriers to care due to lack of access or social stigma and may have significant medical and psychosocial complexity.
  • Areas of clinical focus: HIV/AIDS, viral hepatitis, healthcare for the homeless, substance use disorders, LGBTQIA health and mobile health delivery.

Physical Requirements/Work Environment

  • Hours as agreed; Clinical schedule arranged to include office hours, hospital rounds, etc. One evening office session per week, and one weekend session per month. On-call schedule as required by needs of practice and established in contract
  • Moderate physical activity level, i.e. walking, standing
  • Potential bloodborne pathogen/chemical hazard exposure
