General Description
The Nursery Navigator (Navigator) is primarily responsible for coordinating the transition from hospital to Evara Health for both the mom and baby. The Navigator provides education on the importance of post-partum and newborn care after the hospital and related patient education. The Navigator is responsible for registration and scheduling of patients and identifying and removing barriers to care.
Essential Duties & Responsibilities
Work collaboratively with hospital partner and team members
Builds and establishes trusting relationships with Evara Health patients through ongoing outreach and education thereby improving individuals’ physical and mental health (measured by a reduction of hospital ER visits and hospital admissions, satisfaction surveys, medical compliance). Advocates for individuals and their family members within the community, as needed
Coordinates care transition of newborns from hospital to Evara Health’s pediatric team
Coordinates postpartum visit to Evara Health’s women’s health team or family medicine.
Assess Social Determinates of Health and coordinate appropriate resources to remove barriers.
Provides ongoing patient education and advocates for patient needs with physicians and medical services.
Links the individual/family with necessary resources to assist the family with needs and monitors the effectiveness of interventions being provided.
Meet daily with each Evara Health Patient providing education and resources and documenting the same.
Other duties as assigned.
Strictly adheres to all Evara Health, HIPAA, HRSA, State, Federal and accreditation agency rules and regulations.
Education and Experience
Highschool Graduate
Minimum Qualifications
Organized with excellent attention to detail skills
Ability to effectively navigate and enter data into an EHR
Exceptionally outgoing and enjoys working in the hospital setting with moms and babies
Strong verbal and written communication skills
Excellent communication and interpersonal skills and ability to develop and maintain effective working relationships with business partners and coworkers
A self-motivated, independent, highly driven and results focused individual
Essential Physical Skills
Able to walk, stand, talk, use hands to finger, handle or feel, reach with hands and arms, climb and descend steps, carry or lift up to 25 lbs., stoop, kneel, crouch and/or crawl.
Able to read up close or at a distance, identify and distinguish colors, have peripheral vision (up, down, left, right, while eyes are fixed on a given point), and have depth perception (3‑dimensional vision, judge distance and spatial relationships).
Able to communicate, verbally and in writing with patients, vendors and/or co-workers.
