ABOUT THE ROLE :
Community Health is seeking a Psychiatric Nurse Practitioner to provide outpatient psychiatric services to a diverse population in Rutland County. Must e njoy working with a dedicated team of highly motivated professionals who are committed to providing an integrative, community-based approach to care.
You will be involved in diagnosing, treatment and prevention of behavioral health disorders. Our medical staff work with a team of case managers, therapists and support staff to ensure the best possible outcome for our patients.

Community Health Center sites are certified Level 111 Medical Homes, with a highly developed and supportive panel management and case management system in place.
