What are the responsibilities and job description for the Intensive Care Manager position at Community Health Network of Connecticut, Inc.?
Community Health Network of Connecticut, Inc. (CHNCT) is currently seeking five (5) Intensive Care Managers to join our team!
Primary Responsibilities:
- Collaborates with CHNCT’s clinical team, provider network, DSS and the community to create a process of assessment, planning and facilitation to meet the member’s heath care needs across the continuum of care.
- Assists the member by functioning as an advocate for options and services by communicating effectively with other members of the healthcare team and by utilizing available resources to promote quality cost effective outcomes and member autonomy.
Tasks Performed:
- Develops, monitors and evaluates the effectiveness of the care plan and modifies as necessary.
- Provides education and coordination of outpatient care to CHNCT members selected for Case Management.
- Identifies and refers quality of care, coordination of benefits and third party liability issues as appropriate.
- Orients new staff as requested.
- Performs other duties as assigned.
Essential Functions:
- Engages with members through telephonic, videoconferencing and face-to-face outreach; meets with providers and community organizations to coordinate care.
- Conducts assessments to identify the member’s needs and develops a specific care plan to address objectives, barriers and behaviorally specific goals identified during the assessment.
- Maintains knowledge of covered benefits for CT Medicaid, HUSKY Health programs.
Desired Education: Two to four years post-secondary schooling required
Desired Degree: Associate's degree, Bachelor's degree preferred
Desired Major: Nursing (required)
Desired Job Experience: Minimum two years direct clinical care experience as an RN is required. Behavioral health experience and knowledge of case management are beneficial.
Certifications Required: Active Registered Nurse License, State motor vehicle license
