We are seeking a dynamic and compassionate Social Services Manager to join our team in providing direct care and support to individuals transitioning from the justice system. This role is integral in ensuring that incarcerated individuals receive the necessary social services, healthcare, and community resources to support their successful reintegration into society. The Social Services Manager will oversee case management, coordinate care, and empower individuals to navigate healthcare systems, ultimately working to reduce barriers to care and improve outcomes for justice-involved populations.
Key Responsibilities :
Direct Support Within the Jail Facility :
Build strong, trusting relationships with incarcerated individuals, assess their health and social needs, and facilitate access to medical care and services. Work closely with inmates to develop and implement discharge planning to ensure successful transitions after release.
Case Management :
Develop and implement individualized care plans for each patient, addressing their unique healthcare, mental health, substance use treatment, housing, and legal needs. Serve as a consistent point of contact for individuals throughout their time in the justice system and into their reintegration phase.
Care Coordination :
Coordinate services both within the jail and after release to ensure continuity of care. Collaborate with healthcare providers, community organizations, and support systems to ensure that individuals receive the resources they need for successful reintegration into society.
Empowerment and Independence :
Empower individuals by teaching them how to navigate healthcare systems, understand their medical conditions, and take ownership of their health. Work with patients to foster self-sufficiency and resilience as they transition from incarceration to community living.
Resource Connection :
Connect individuals with essential community resources, including healthcare, housing, legal aid, substance use treatment, mental health services, and other support systems. Help break down barriers to accessing these services, ensuring that patients are supported in achieving their goals.
Community Reintegration :
Provide guidance and support as patients reintegrate into society after release, ensuring they have the necessary tools to succeed. This includes securing housing, employment, healthcare, and social supports to ensure long-term stability and reduce the risk of recidivism.
