DESCRIPTION:
The Family Team Consultant will conduct coordination of care for children and families referred from schools and various agencies to provide intake assessments and engage families in the development of individualized child/family care plans that will link them to services that address the full range of their needs. Attends orientation sessions, scheduled meetings, and face-to-face consultation with supervisor. Consistently gives attention to detail. Coordinates Family Team Conferences and intake assessments for all families referred to program. Assist families to access the health care system and assign to a medical home. Communicate effectively and expresses ideas clearly; actively listens and always follows appropriate channels of communication. Maintains flexibility to access families during non-traditional hours. Collects and report results of the Family Team Conference and intake assessments. Develops Care Plan for team to follow. Collects specific data as required to meet the goals of the program. Maintains accurate records of the activities performed. Follow-up and track missed appointments. Track coordination of service and needs based on care plan. Attends scheduled in-services and trainings. Works to achieve patient outcome goals as established in the Scope of Service for Service Partnership and as required by any other funding source. Follows CHI Policies and Procedures. Maintains 80% productivity monthly. Reports to work on time and ready to work with minimal absenteeism. Adheres to Confidentiality Policies and Procedures / HIPAA Regulations. Performs other duties as assigned.
QUALIFICATIONS:
Graduate from an accredited college or university with a minimum of a Master’s Degree in Social Work or Mental Health Counseling; or eligible to become a Registered Intern with the Florida Board of Health in Mental Health Counseling or Social Work. Two (2) year’s experience in a health care or related setting. Experience in program development, project planning and implementation. Experience working with culturally diverse clients. Knowledge regarding the Joint Commission and other regulatory agencies requirements. LMHC and LCSW preferred but not required. Maintain current CPR certification from the American Heart Association. Must have a valid Florida Driver’s License. LMHC and LCSW preferred but not required. Excellent communication and interpersonal skills. Sensitivity in working with diverse cultural groups. Ability to present ideas and recommendations clearly and concisely in writing and orally. Excellent interpersonal communication skills; Proven ability to problem solve and solution oriented. Strong ability to work independently and as a team member with fellow co-workers, supervisors and other staff. Demonstrates flexibility in work schedule and job assignments. Strong computer skills required.
