Description
The Peer Recovery Coach / CHW position is contingent upon grant funds, employment will depend on the successful acquisition and continuation of the behavioral health grant.
Set Salary : $28.00 / per hour
The Peer Recovery Coach / Community Health Worker will provide non-clinical support, education, and advocacy to patients navigating recovery from substance use disorder (SUD), alcohol use disorder (AUD), psychiatric illness, and / or chronic medical conditions. Drawing on lived experience and / or professional expertise, the individual will foster trust, build rapport, and empower patients to achieve recovery goals while accessing comprehensive health care and social service support. The Peer Recovery Coach / Community Health Worker will participate in a robust collaborative team comprised of two medical providers with addiction and primary care expertise, a clinical psychologist with addiction expertise, and a nurse manager, working out of a mobile health unit and brick and mortar sites to engage patients who have had trouble entering regular health care. This person will support CHP's mission, vision, and values and will adhere to compliance protocols as well as CHP's policies and procedures.
Position Requirements
Peer Recovery Coach / Community Health Worker Essential Duties and Responsibilities :
- Connects with patients to build trust and rapport, to better understand patients' goals, and to help patients access behavioral health, medical, and social services. Helps guide and inform the team about the patient's goals and needs - so the team can focus on meeting the patient where they are at.
- Completes a grant intake for new patients by meeting one-on-one reviewing social needs and entering the answers into the grant database. This will typically not be done on the first visit, but more likely on the 2nd or 3rd visit. The state will provide gift cards to patients to incentivize completion of this data collection.
- During clinical sessions on the Mobile Health Unit (MHU) :
Provides situational awareness, and along with the other MHU providers, assesses scene safety.
Reaches out to individual patients who need recovery coach assistance via the phone, a pre-arranged visit at one of CHPs brick and mortar sites, or on occasion a home visit with other members of the team for emotional / peer support, guidance on accessing recovery and community support and services, and when time allows, accompaniment to important events or appointments.
