Community Health Resource Center (CHRC) is a nonprofit 501 (c) (3) corporation. Since 1986 CHRC’s mission has been to provide and promote innovative health care and health maintenance programs to the greater Bay Area as well as the patients served by the California Pacific Medical Center. CHRC provides counseling, education and coordination of multiple patient services to supplement the care and support provided by physicians.
Position Description: Under the direction of the Executive Director, the Licensed or Associate Clinical Social Worker position provides responsive, comprehensive assessments, community based, and outpatient medical social work services to individuals, families and groups. This position will include working directly with advance organ therapies (liver, heart and kidney transplant) medical teams in the capacity of medical social worker. This position is flexible, customer service focused and highly skilled in assessing and serving the biopsychosocial needs of clients and medical center. The social worker works collaboratively with multiple teams in an outpatient environment and the community to support emotional, physical and financial health. Involvement in program development and marketing for community based services including but not limited to social work services. Offer health education lectures to community members and participating in several disease prevention and early detection programs organized by CHRC. Possible to provide direct medical social work services to several hospital departments affiliated with CHRC.
Required Skills
· Ability to handle a variety of tasks simultaneously
· Provide individual and group psychotherapeutic services
· Excellent organizational and time management skills
· Demonstrated capacity to work with a culturally diverse population
· Ability and willingness to maintain confidentiality
· 2 years of experience providing direct services to individuals with advanced illnesses, chronic or acute health problems
· Demonstrated understanding of and experience working with mental health issues and their effects on individuals and families.
· Knowledge of medical arena, current resources and service/benefits programs in San Francisco and the Greater Bay Area
· Superior record keeping
· Knowledge of organ transplant psycho social processes and patient needs
· Other duties as assigned
Education and/or Experience:
Licensed or Associate Clinical Social Worker in the state of California, in good standing with the board of behavioral sciences of California, Masters in Social Work required. Licensed and License eligible candidates encouraged to apply.
Computer Skills:
To perform this job successfully, an individual should have knowledge of Microsoft Office, Outlook software; database management preferred and electronic medical record documentation processes.
