ABOUT THE POSITION: BHH Wellness Specialist/Health Coach
Leads a comprehensive and engaging wellness program to improve the health outcomes and increase the independence and self-efficacy of individuals with serious and persistent mental illness and chronic health conditions through outreach, support, coordination with medical services, direct assistance and training in living skills, advocacy, and by providing education, training, and oversight of physical wellness activities.
By:
- Working with adult and/or child clients as assigned.
- Providing individuals with serious and persistent mental illness and chronic health conditions assistance with achieving and maintaining their highest level of health.
- Assisting individuals in linking to community resources and entitlements.
- Linking individuals with appropriate treatment sources.
- Coordinating care with medical and collateral providers.
- Assisting individuals with accessing health care services, including transportation as required.
- Assisting with hospital and other care transitions.
- Providing health and wellness education and training, in individual and group formats.
- Arrange and coordinate health and wellness related classes and expert speakers for client education.
- Assisting individuals with setting and meeting wellness and recovery goals and managing chronic illness.
DUTIES & RESPONSIBILITIES:
- BHH Specialists and/or case managers working with adult and/or child Behavioral Health Home clients provide BHH services including:
- Comprehensive Care Management
- Care Coordination
- Health Promotion
- Comprehensive Transitional Care
- Patient and Family Support
- Referral to Community Support Services
- Collaborates with other BHH and CHR team members to provide improved medical and behavioral health care coordination and outcomes for BHH enrollees.
- Works to reduce health care disparities whenever possible.
- Demonstrates competency in age related or specialty issues and developmental needs for each population served.
- Assists in obtaining access to medical and wellness services including tobacco cessation intervention services, benefits, housing, treatment, legal services, medical services, education and employment, as needed.
- May provide client transportation as needed.
- Provides high level of collaboration with medical providers to ensure medical follow up including PCP & specialist visits, labs, test and procedures to manage chronic health conditions.
- Participates in hospital discharge process to promote highly coordinated care transitions for behavioral health and or medical admissions, with the aim of reducing the likelihood of readmission.
- Provides wellness and health-promoting lifestyle interventions such as, but not limited to nutritional counseling, smoking prevention and cessation, obesity reduction, increasing physical activity.
- Provides crisis intervention and support as clinically indicated.
- Works as liaison between agency and state and private hospitals, other community-based services, family members and significant others.
- Maintains documentation in compliance with agency and program standards.
- Participates in staff meetings, supervision, trainings and other meetings as assigned.
- Compiles statistical data related to health outcomes of participants.
- Assessing and evaluating program effectiveness and making recommendations to the program director.
- Additional duties as required.
QUALIFICATIONS:
Education: BA/BS in human services, health education or related field required.
Experience: Two years experience in mental health setting. Experience working in integrated care setting preferred.
Licensure/Certification/Registration: Personal Training or Group Fitness Certification or similar. CPR/First Aid within first 6 months, Valid driver’s license.

COMPENSATION*:
$18.86/hour min min, and up commensurate upon experience.
- Actual rates are determined at the time of offer and are based off relevant experience, as well as adherence to our internal equity policy. Many of our positions offer program/location differentials and/or sign-on incentives. (Please note, for part-time positions this range would be pro-rated based on hours worked)
