Job Summary
We are seeking a highly skilled Clinical Healthcare Specialist to join our team at Community Health Resources. As a key member of our interdisciplinary treatment team, you will provide comprehensive care management for adults with substance use and mental health disorders.
Responsibilities
- Develop and implement individualized treatment plans that address the unique needs and goals of each client.
- Collaborate with clients, family members, and other healthcare providers to ensure seamless care coordination and transitions.
- Provide education and support to clients on coping skills, ADLs, and psychoeducation to promote recovery and wellness.
- Maintain accurate and timely records of client progress and treatment outcomes.
- Serve as a resource and advocate for clients in need of additional support or services.
Requirements
- Graduate of an accredited School of Nursing with RN licensure.
- Experience working with patients with substance use and mental health disorders.
- Strong communication and interpersonal skills with ability to work effectively with diverse populations.
- Proficiency in electronic medical records and other relevant software applications.
- Ability to maintain confidentiality and handle sensitive information with discretion.
