About Our Program
Hendee Road Home is a 6-bed residential program designed for men with severe mental illness who are transitioning from long-term hospitalization. Our program focuses on empowering individuals to increase their independence and self-efficacy through round-the-clock support in both the residence and community.
We prioritize teaching essential life skills, such as symptom management, coping mechanisms, medication management, healthy relationships, personal safety, financial literacy, substance use education, recovery support linkages, and sober activities. In addition, our staff provides crisis intervention to minimize hospitalizations and emergency room visits.
This position requires a Bachelor's degree in human services and excellent communication skills to maintain family and collateral contacts, ensuring quality client-centered services. Assessments and involvement of natural supports in treatment planning and discharge processes are crucial aspects of this role.
Responsibilities
- Maintain accurate documentation of all services and contacts using our Electronic Medical Record (EMR)
- Collaborate with multidisciplinary teams to develop individualized treatment plans
- Provide education and support to clients and families regarding mental health conditions, medications, and daily living skills
