Pediatric and Adolescent Medicine

Non-exempt, Full Time Position

Community Health Services, Inc. is seeking a Care Coordinator for our Pediatric and Adolescent Medicine Department. The incumbent's primary responsibility is to consistently provide patient-centered administrative support to the department in patient care coordination processes. Different tasks included in the Care Coordinator role are related to home care orders, prior authorizations, scheduling hospital discharge follow up appointments and developing and implementing Population Health Management Initiatives. Position reports directly to the Pediatric Nurse Manager. This multi-disciplinary work will be in alignment with Community Health Services' Organizational Core Values and the Patient Centered Medical Home approach to care.

The successful candidate will :

  • Coordinate referrals for medical services as necessary with external providers and assist patients with pre-visit planning, including prior authorizations, transportation needs, confirming appointments, obtaining medical records, and completing any other necessary paperwork or activities to eliminate barriers to care
  • Compile and provide patient records to external providers as required complete referrals
  • Ensure that referrals are addressed in a timely manner by submitting, processing, tracking, documenting, and following up on all referrals requested by providers
  • Collaborate with medical records to ensure reports, test results, and other documentation is captured in the electronic health record
  • Ensure closed-loop communication through correspondence with patients and with internal and external providers regarding appointment time, location, directions, and the overall status of referral
  • Timely document care coordination activity, including referrals, hospital admission, outcomes of services delivered and other information, into electronic health record as appropriate
  • Communicate with patients to identify and provide assistance in resolving potential barriers to care, including financial issues and issues with language and / or transportation
  • Facilitate a smooth transition into the organization from inpatient or emergency department visits by using various databases to obtain and review admissions and discharge summaries and working with reception staff to schedule appointments
  • Assist patients in attaining access to care and services, by advocating for patients with insurers, providers, and governmental agencies
  • Maintain knowledge of health care services and providers in the community and the resources available to support patients' social determinants of health needs
  • Develop and maintain positive relationships with partners in the community who can provide healthcare and other services to patients
  • Participate in outbound calling as assigned or as necessary to confirm appointments or bring patients into care
  • When needed, act as backup resource for incoming telephone calls to the nurse, addressing, transferring, and otherwise managing calls as appropriate
  • Communicate with patients in their preferred language, using interpretive services as necessary, and interpreting for provider if proficiency in language has been established
  • Contribute to population health efforts by extracting and analyzing patient utilization information from the electronic health record, data analytics software, and other systems
  • Perform all duties in accordance with CHS Core Values, Compliance Program, and other policies and procedures
  • Participate in quality and process improvement efforts in the department and the organization as requested or as directed
  • Complete training and professional development requirements in a timely manner
  • High school diploma or equivalent
  • CPR certification at the healthcare provider level or eligible to obtain certification within 90 days of hire
  • Proficiency in the use of Microsoft Office applications, including Word, Excel, and Outlook
  • The ideal candidate will also be a Medical Assistant
  • Spanish speaking candidates preferred
