POSITION SUMMARY
Community Health and Wellness Partners (CHWP) is an integrated primary care community health center
that utilizes a team based care model, sharing patientcare responsibilities among members of a team.
The Patient Care Assistant is responsible for maintaining an excellent attitude in participating of the dayto
day clinical operations of the health center under the guidance and support of the Clinical Coordinator.
Responsible for the collection of data, administration of medications and treatments, and documentation
of care provided as qualified. Supports the coordination of all activities of team members engaged in
clinical care for patients under the directions of the Clinical Team Lead, Clinical Coordinator and/or
Providers. Responsible for training like staff in teambased care and supports team members to stay
focused on the organization’s Mission, Vision and Values.

- Ensure all actions, job performance, personal conduct and communications always represent
- Uphold and ensure compliance and attention to all corporate policies and procedures as well as
- Compliant with HIPAA.
- Volunteer to serve the community by offering five (5) hours annually to a community service
- Participates as a member of the teambased care model by being a strong team builder, working
- Provides direct nursing or medical assistant care to patients as assigned and qualified to perform
- Supports care plans to ensure maximum patient outcomes by working with team members and
- Completes all patient charting, and ensure all patient records are current.
- Participates in quality strategies to evaluation compliance with standards and identify
- Attends and participates in all meetings, patient conferences, planning sessions related to
- Attends continuing medical education to maintain best practices and licensure or certification

WORK ENVIRONMENT
The work environment characteristics are those for a normal community health center environment
(ambulatory primary care office). Reasonable accommodations may be made to enable individuals with
disabilities to perform the essential functions. Work may require occasional weekend and/or evening
hours.
Work hours
Fulltime, nonexempt hourly position. Generally, a 40hour work week is required for fulltime as
agreed upon by parties. Hours are subject to change. Office hours vary by site, but are usually 8:00
am to 7:00 pm, Monday through Friday. Some Saturdays and after business hours work may be
required. Assigned meetings and travel are a part of the position.
Wages
Starting wages for Patient Care Assistant will be negotiable based on Community Health & Wellness
Partners pay policies and factors such as education and experience, national and state average
compensation recommendations based on location and region.
Introductory Period
Ninety (90) calendar days probationary period. The successful completion of the introductory period
does not alter the at will employment status.

Education
- Registered or Licensed Practical Nursing Degree; or
- Medical Assistant Certification/Licensure; and/or
- Three to Five years of experience nursing in an ambulatory health care setting, health center or
Certificates and Licenses
- Valid Ohio driver's license.
- Active Registered or Licensed Practical Nursing License within the State of Ohio; or
- Current or active Medical Assistant Certification.
- Demonstrates expertise in relevant medical practices, protocol, trends and best practices in
- Demonstrates knowledge and success in effecting overall clinical operations.
- Experience/Knowledge of accreditation processes and requirements, as well as all federal, state
center environment.
- Ability to manage multiple responsibilities and emergency situations successfully
- Proficiency in knowledge of office management, use of computer, software packages, and office
Physical/Environmental
- Occasional lifting of 40 – 50 lbs. and pushing of 520 lbs.
- Sitting, standing and walking approximately 80% of the time.
- A medium to high level of manual dexterity required.
- Bending and reaching approximately 20% of the time.
- Normal accessibility and mobility throughout the region required.
- Normal overtime/extended work hours.
