POSITION SUMMARY
Assist the RN Case Manager in the plan of care for the patient.
- Assists RN in case management of total plan of care of patient/support persons by coordinating with team members and communicating with IDG in accordance with organizational policies, procedures and regulations that define the scope of practice.
- Complete, maintain and submit accurate and relevant clinical notes regarding the patient’s condition and care given. Record pain/symptom management changes/outcomes as appropriate.
- Administer medications and treatments as prescribed by the physician.
- Document nursing care and significant interactions with patients, support persons, and team members.
- Assess patient/family response to care and participate in follow-up with bereaved families with various types of contact.
- Manage assigned patients equally, regardless of severity, contagion, or complexity of diagnosis.
- Participate in on-call rotation
POSITION QUALIFICATIONS/REQUIREMENTS
Licensed Vocational Nurse licensed by the State of Texas
Hold a valid driver’s license and have a dependable means of transportation
1 year nursing, hospice, or oncology experience preferred
1 year LVN experience in an acute care setting required.
Proficient skill using a computer is required, Microsoft office suite experience preferred.
