What are the responsibilities and job description for the Hospice Nurse Practitioner position at Community Healthcare of Texas?
POSITION SUMMARY
Works in collaboration with the Chief Medical Director and Hospice Team to provide Hospice care services to geriatric and pediatric patients and families within the scope of practice of a nurse practitioner. Perform procedures for which he/she is privileged to do and document assessments, evaluations, consultation reports, and progress notes.
ESSENTIAL FUNCTIONS
- Conduct Face-to-Face visits to assess patients' need for ongoing hospice eligibility.
- Complete required documentation to support eligibility and to meet regulatory requirements for communication of finding to Community Hospice of Texas / Providence Hospice Medical Director.
- Communicate to RN Case Manager any patient needs requiring medical intervention.
- Coordinate visits and scheduling with the Patient Care Manager.
- Maintain compliance with all regulatory and accrediting agencies.
- Responsible for demonstrating an appreciation and understanding of the philosophy, mission, and service standards of Community Hospice of Texas and for providing excellent customer service.
POSITION QUALIFICATIONS
- Licensed as an Advanced Practice Nurse in the State of Texas
- At least 2 years of experience in geriatrics or hospice
- Excellent organizational skills
- Proficient skill using a computer is required; Microsoft Office Suite experience is required.
