POSITION SUMMARY
Assist the RN Case Manager in the plan of care for the patient.
- Assists RN in case management of total plan of care of patient / support persons by coordinating with team members and communicating with IDG in accordance with organizational policies, procedures and regulations that define the scope of practice.
- Complete, maintain and submit accurate and relevant clinical notes regarding the patient’s condition and care given. Record pain / symptom management changes / outcomes as appropriate.
- Administer medications and treatments as prescribed by the physician.
- Document nursing care and significant interactions with patients, support persons, and team members.
- Assess patient / family response to care and participate in follow-up with bereaved families with various types of contact.
- Manage assigned patients equally, regardless of severity, contagion, or complexity of diagnosis.
- Participate in on-call rotation
POSITION QUALIFICATIONS / REQUIREMENTS
Licensed Vocational Nurse licensed by the State of Texas
Hold a valid driver’s license and have a dependable means of transportation
1 year nursing, hospice, or oncology experience preferred
1 year LVN experience in an acute care setting required.
Proficient skill using a computer is required, Microsoft office suite experience preferred.
COMPETENCY STATEMENT / EXPECTATIONS
PHYSICAL REQUIREMENTS :
While performing the duties of this job, the employee will be required to stand, walk, lift, carry, push, climb, pull, bend, twist, squat, reach and kneel, on a daily basis, for varying lengths of time according to each patient’s need. Employee must be able to carry up to 25 pounds and lift up to 50 pounds.
Work Environment Requirements
While performing the duties of this job, the employee may work in various locations (homes, facilities, hospitals, Inpatient Units) and in various conditions with possible exposure to blood and bodily fluids and to infectious diseases. The job requires the ability to work a flexible schedule; ability to travel locally; PRN emergency call. While performing the duties of this job, there may be exposure to outside weather conditions when traveling between sites. Patients’ home environments are variable regarding heat, noise, air circulation and conditioning, space, cleanliness, accessibility and all other environmental factors. Inpatient and skilled nursing facility environments are standardized. Office environment is in a controlled atmosphere building.
