Position Summary

Meet with patients and families to explain the scope of hospice services, coordinate admission of patients, and ongoing plan of care.
Essential Functions
- Assess patient’s physical condition according to Plan of Care; determine and/or revise plan of care regarding medications and DME.
- Case manage total care of the patient by coordinating with team members/physicians, supervising C N A activities, and participating in staff and Interdisciplinary Team meetings.
- Complete daily paperwork and charting, thoroughly and accurately according to Policy.
- Participate in on-call rotation.
- May assist with admissions
- Registered Nurse in the state of Texas
- Hold a valid driver’s license and have a dependable means of transportation
- 1 year of nursing experience
- Case Management experience (hospice)
- 2 years of experience in hospice case management strongly preferred
- Proficient skill using a computer is required; Microsoft office suite experience is preferred.
