What are the responsibilities and job description for the Behavioral Health Nurse Practitioner position at Community Healthcare Partners?
The Psychiatric Mental Health Nurse Practitioner (PMHNP) will provide outpatient psychiatric evaluation and medication management services at CHP. The PMHNP will work in coordination with CHP's outpatient primary care and in 2025 counseling services to provide psychiatric services to our primarily geriatric population patients with behavioral health issues (dementia, depression, anxiety, PTSD). This position will be supervised externally by a psychiatrist with whom there is a Practice Agreement established. The PMHNP will work out of Omaha, Fremont and Lincoln metro areas. Services will initially be home based, Telehealth, facility based, or in office. The majority of patients will be seen in their home facility. Telehealth accounts for a good number of the at home visits. The PMHNP will work in collaboration with office staff & Triage Nurse to coordinate client care, assist with insurance pre-authorization, and caseload management.
Work Schedule Options:
- 5 8hr shifts.
- 2 Days will be completed in Lincoln Area and 2 Days will be in the Omaha area (mileage paid/drive time paid)1 day in Fremont Area
- Some weekend required

This position has a bonus structure in place, allowing you unlimited earning potential. 10 visits/ day = expected salary w/ base bonus ($120,000 base plus RVU points keeping pay in the $175,000 range) No cap for additional visits/work!! Responsibilities
- Examine patients and their medical records
- Patient population is largely Geriatric (Dementia, Depression, Anxiety,PTSD)
- Order and study diagnostic tests (blood tests, X-rays etc.)
- Diagnose health conditions and illnesses
- Propose treatments for chronic and infectious diseases
- Prescribe and manage medications
- Maintain accurate electronic records and schedules
- Educate patients and their family members.
- Patients are seen in residential facilities, in their homes, and through telehealth.
- Oversee patients’ overall health
- Promote good health practices
- Advance professional knowledge and conduct research
- Collaborate with medical director and other providers
- 2 Year Signed Commitment
- Will cover Lincoln/Omaha/Fremont areas
Skills/Requirements/Qualifications
- Masters's or Doctorate Degree in Nursing with focus on Psychiatric nursing Required
- Graduation from a state certified advance practice program specializing in psychiatry/mental health
- Proven experience as a NP in a clinic, hospital, ER or other setting. Both Behavioral and Primary Care experience required.
- 1-2 years of experience REQUIRED , providing client care to persons receiving treatment for psychiatric disorders
- Passion for Behavioral Health

- Valid NE NP license
