POSITION SUMMARY
Provide direct patient care duties under the supervision of licensed nursing personnel and to assist in maintaining a positive physical and psychosocial environment for the patients.

Essential Functions Statement(s)
- Assists patients with activities of daily living including bathing dressing, grooming, toileting, changing of bed linens, and positioning in and out of bed, chair, monitoring I & O, rounding on patients to ensure safety.
- Assists with patients' recreation programs.
- Prepares patients for meals and snacks, assists patient in eating where needed, and records food intake.
- Reads and follows daily care plans.
- Reports changes in patient condition to nurse in charge.
- Documents care provision on patient record / flow sheets as required and reports accidents and incidents.
- Provides nursing functions as directed by licensed nurse, supervisor or manager.
- Understands, complies with, and promotes all rules regarding patient rights.
- Promotes positive relationships with residents, visitors and government regulators, to include presenting a professional appearance and attitude.
- Assist in admission, transfer and discharge of residents.
- Supports and abides by the mission, values, and policies of CHCS in all activities.
- On-going compliance with CHCS educational requirements.
- All other duties as required.
