What are the responsibilities and job description for the Linkage Specialist | Longview position at Community Healthcore?
Job scope: Participates
in a multi-faceted team that strives to engage females into treatment
services. Activities include service
coordination, linking to community resources and encouraging the participants
to engage into and complete treatment.
Will partner with community referral agencies and treatment providers
and organizations. Coordinates with CCC
team to identify needs and will strive to meet needs. Builds relationships with community partners
that serve pregnant and parenting women.
Demonstrates knowledge of chemical dependency and abuse issues,
dynamics, and impact on family and kids.
Provides services in a trauma -informed, culturally competent, and
developmentally appropriate manner.
Provides services through tele-video, phone and in-vivo platforms. Ability to calmly problem solve, interact
with, and communicate with individuals that are negatively impacted by drug and
alcohol use and abuse.
Education/licensure: High School Diploma, prefer Bachelor's Degree
with SUD/human services emphasis. Current valid driver's license in state of
residence.
Experience/Knowledge: Knowledge and experience in family treatment and chemical dependency. Must demonstrate effective oral and written communication skills, problem solving skills, organization skills, and team building skills. Typing and computer skills sufficient to meet and maintain documentation requirements. Must demonstrate public speaking skills. Prefer previous management experience.

qualifications essential to the performance of the job: Have no uncorrected deficiencies in the senses,
particularly in sight, smell and hearing. Mobility and ability to bend and
reach during an 8 - 10 hour day. Able to lift a minimum of 10 lbs. Sensory
acuity sufficient to evaluate, treat and monitor people's needs and maintain
accurate and current documentation of service provided. Fine motor skills
sufficient to legibly and accurately complete reports and records. Endurance to
work an 8 - 10 hour day.
