Overview
Community Physicians provides primary care, as well as psychiatric medication management services to residents residing in assisted and long term care settings. We strive to provide the highest quality of care for our patients, with our Nurse Practitioners providing services within the facilities . This relieves our patients of the worry of traveling and keeping track of appointments. Our Care Coordinators play an important role in the quality of care, as they serve as the bridge between our providers, facility care teams, and patients. This also allows for our providers to focus on patient care during the business day.
Care Coordinator Responsibilities
- Coordinate daily with healthcare teams, ensuring efficient patient care and communication.
- Processing medication and lab orders.. Ensure accurate communication of medication queries and lab results, enhancing operational accuracy and efficiency.
- Data entry of patient demographics, medications, diagnosis.
- Manage facility census with precision. Develop and implement schedules for nurse practitioners, improving visit organization, as well as updating the provider’s follow up visits.
- Uploading, emailing, faxing nurse practitioners visit notes to the appropriate facility in a timely manner.
- Process and maintain records with compliance of HIPAA.
- Foster seamless coordination among healthcare professionals, ensuring timely patient care. Provide kind, professional support to patients and families.
