Shift: Monday-Friday 3pm-11pm (40 hrs)
Assisting veterans in our Service Intensive Transitional Housing model in preparing for permanent community-based housing. The Case Manager provides case management and support services to veterans with the goal of improving financial stability, increasing self-determination; developing positive coping strategies; and finding affordable, safe, and stable permanent housing in the community.
Responsibilities:
- Provides non-clinical case management services including intake, assessment, housing stabilization, budgeting, financial entitlements/benefits, etc.
- Develops Individual Recovery Plans (IRP) with veterans that adhere to VA Grant & Per Diem guidelines.
- Provides supportive counseling for veterans in need of crisis intervention and emergency services.
- Assists in the administration of drug and alcohol screenings.
- Supports veterans in their recovery through psychoeducation groups, housing search and placement, and linkage to community-based services.
- Provides housing search services and support to all veterans with an upcoming discharge date including outreach and advocacy, linkage to benefits and subsidies, coordinates move in and securing necessary household essentials through low cost connections, and discharge planning.
- Develops and nurtures relationships with community providers and landlords invested in supporting veterans.
- Function as a team member including emergency shift coverage, communicating all relevant information to other team members, covering co-workers’ duties as needed.
- Transports veterans, as needed, in agency vehicle.
- Documents service delivery and maintains accurate, timely, and detailed records.
Qualifications:
- Bachelor’s degree in Psychology, Social Work, Human Services, Criminal Justice, or a related discipline; OR two years of college plus two years of related work or life experience including experience as a veteran; OR a High School diploma and four years of related work or life experience including as a veteran, in homelessness, mental health or recovery services.
- 1-year of prior experience in housing services required, preferably in working with veterans.
- Prior experience in community outreach, marketing, or communication preferred.
- Bi-lingual preferred; Diversity welcomed.
- Must possess a valid driver’s license, with a good driving record.
