We are looking for a full-time, on-site (four days per week in-person) Licensed Mental Health Clinician to assess, authorize, and complete individual rehabilitation plans for adults with serious mental illness.
- Performs a comprehensive rehabilitation needs assessment.
- Contributes to the development, implementation, monitoring, and updating of the individualized rehabilitation plan.
- Provides clinical support and on-call coverage to staff and consumers for crisis intervention and emergencies.
- Support consumers who may be experiencing symptoms related to mental illness and / or substance use.
- Maintains compliance with all regulatory, contractual, accreditation, and quality standards including the Agency Performance Management Plan, policies and procedures, and corporate compliance standards.
- Follows Agency financial procedures as it relates to the submission of individualized rehabilitation plans.
- Documents service delivery accurately and timely while ensuring consumer confidentiality.
- Organizes, attends, and participates in team, supervision, and training meetings.
- Promotes wellness and recovery practices, a consumer choice focus, and valued life roles for each individual served.
