What are the responsibilities and job description for the Outreach Coordinator position at Community Hope, Inc.?
A full-time exempt position conducting outreach to providers in the community serving veterans and/or their families in order to increase referrals to and engagement in the Bring Home Veterans Program (BVH). Flexible hours that may include evenings and weekends and an ability to travel within the service delivery areas are required.
- Conduct regular outreach in areas where homeless veterans are known to congregate and to assist with connecting them to services. This may include ability to traverse identified regions including camps, parks, under bridges, motels, shelters, meal sites, libraries, transit hubs, vehicles, and other outdoor or unsafe locations to locate and establish rapport with unsheltered veterans.
- Perform initial assessments to understand specific needs and vulnerabilities and enter data into NJ HMIS.
- Coordinate with emergency shelter, diversion, and housing navigation programs.
- Participate in case conferencing and By-Name List processes.
- Provide transportation assistance to connect veterans with services and housing opportunities
- Coordinate with other program components to ensure a seamless transition to Housing.
- Attend County Continuum of Care, Stakeholder, and other relevant meetings in the community to market program, which includes presentations to stakeholders and interested parties.
- Represent BVH at conferences, community, and outreach events, which may include some evening and weekend hours.
- Provide additional support as needed to expand outreach and awareness of the BVH Program
- Documents service delivery accurately and timely.
- Ensures the rights of veterans are upheld in the program including confidentiality.
- Function as a team member by communicating all relevant information to other team members and covering co-workers’ duties as needed.
- Participate in all appropriate team, employee supervision, and training meetings.
- Provides support and direction to Outreach Workers.
- Upholds the highest ethical and professional standards in adherence with our Code of Conduct & Ethics.
- Assumes additional responsibilities as assigned by the Regional Coordinator, Program Director, or Leadership team.
Qualifications:
- Bachelor’s degree from an accredited college or university in a non-profit or human services management, mental health, substance use or related discipline.
- Master’s Degree in Social Work, Criminal Justice, Psychology, Counseling, Mental Health, Human Services, or a related discipline preferred.
- Must have crisis intervention experience and/or suicide prevention experience.
- Prior experience in community outreach, marketing, sales, or communication required.
- Must have skill in community resource development, be well organized, a self-starter, able to work autonomously in the community, and able to work as a member of a team.
