What are the responsibilities and job description for the Regional Coordinator position at Community Hope, Inc.?
A full-time exempt position responsible for the oversight and coordination of all Bringing Veterans Home (BVH) Program initiatives within a defined geographic area to end homelessness in the region. Requires flexible hours that may include evenings and weekends and an ability to travel within the service delivery area.
- Acts as the central point of contact for all veteran homelessness efforts in the region.
- Facilitates communication between stakeholders, downflow of information to Community of Care (CoCs), and drives progress towards ending veteran homelessness in the region.
- Facilitates regular case conferencing and engages with stakeholders.
- Manages data and reporting for the region and drives continuous improvement and resource management.
- Coordinates resource development.
- Monitors all aspects of service delivery for accuracy and timeliness including intake, assessment, crisis intervention, outreach, budgeting, financial entitlements/benefits, case conferences, referrals for adjunctive services, family meetings, advocacy, and liaison with other community and governmental agencies, and discharges.
- Ensures eligibility screenings, intake, assessment, contact, individualized service plans, temporary income support assistance, and other information is documented timely and accurately in the electronic health record.
- Assists the Director with public speaking engagements, trainings, and other special events to further promote public awareness of the Program.
- Provides support, supervision, and direction to Case Managers and Outreach Specialist.
- Provides support and coverage to employees, veterans, and their families for crisis intervention and emergency services.
- Conducts team meetings; Participates in committees, supervision, and training meetings.
- Provides employee orientation, training, regular supervision, and completes performance evaluations.
- Transports veterans in agency vehicles in a safe, cautious and responsible manner.
- Ensures the rights of veterans are upheld in the program including confidentiality.
- Upholds the highest ethical and professional standards in adherence with our Code of Conduct & Ethics Participates in team, committee, supervision, and training meetings.
- Functions as a team member by covering co-worker responsibilities, participating in program activities, and communicating relevant information to other team members.
- Monitors and assures compliance with the Agency’s performance management plan.
- Assumes additional responsibilities as assigned by the Program Director or Leadership team.
Qualifications:
- Master’s degree from an accredited college or university in non-profit or human services management, mental health, substance use, or a related discipline.
- One (1) year of case management experience, preferably in working with veterans or the homeless.
- Must have knowledge of clinical issues, skill in community resource development, be well organized, a self-starter, able to work autonomously in the community, and able to work as a member of a team.
- Must possess a valid driver’s license; NJ Driver’s license in good standing.
