Usual Weekly Schedule:

· Monday 3 pm-6 pm

· Tuesday 7:30 am -1:30 pm and 3:30 pm – 6:00 pm

· Wednesday 7:30 – 3:00 pm

· Thursday 7:30 am – 1:30 pm and 3:30 pm – 6:00 pm

· Friday 3:00 pm – 6:00 pm

Pay: $16.32/Hour

Your Tasks will include but are not limited to:

· Providing transportation to community activities

· Dropping off and picking up from work

· Helping maintain a daily routine

· Offering gentle reminders and cues for bathing, showering, nail care etc.

· Encouraging personal care

· Assisting with medications

· Helping out with meal preparation according to dietary needs

You might be a perfect fit if:

· You’re active and love being on the go

· You’re not easily intimated

· Knowledge of seizures is a plus or willingness to learn

· You’re familiar with Autism

· You have a valid driver’s license and reliable vehicle

· You’re comfortable around pets and have no allergies

· You have a friendly nature and enjoy being around others

Who is Community Horizons

Community Horizons offers support for individuals enrolled in self-directed programs through Managed Care Organizations or IRIS (include, Respect, I Self-Direct) throughout Wisconsin. Self-direction gives program participants the choice over who provides services and how they receive them. Community Horizons is not the employer on record, members make all the hiring decisions with the involvement of a third-party fiscal agent responsible for payroll.
