Community Horizons is honored to be assisting with the recruitment process of a self-directed caregiver for an individual enrolled in INCLUSA. Did you know that in self-direction you are hired directly by the individual you are serving? This excellent program allows you to make a mutual decision with the individual to ensure you are compatible and comfortable working together. You won’t find that anywhere else.

Schedule: Pick every weekend or every other weekend.

Friday, Saturday Sunday 8 am -10 am & 8 pm – 10 pm.

Weekday opportunities are Monday, Tuesday, Wednesday and Thursday 8 pm – 10pm. Pick 2 evenings or all 4!

That’s how flexible this position can be!

Pay: $16.75 - $17.20

Location: Stevens Point

Tasks include but not limited to:

- Personal Care such as bed baths

- Sit to stand for transfers

- General housekeeping tasks

- Meal prep and clean up as needed

The right fit for this position:

- Have a valid driver’s license

- Ability to multitask and take direction

- Experience preferred but willing to train

- Cat friendly

- Ability to pass a caregiver background check

- Friendly, reliable and has a great sense of humor

Who is Community Horizons

Community Horizons offers support for individuals enrolled in self-directed programs through Managed Care Organizations or IRIS (Include, Respect, I Self-Direct) throughout Wisconsin. Self-direction gives program participants the choice over who provides services and how they receive them. Community Horizons is not the employer on record, members make all hiring decisions with the involvement of a third-party fiscal agent responsible for payroll.
