Major Duties & Responsibilities
· Incontinence Changing
· Meal Preparation
· Companionship
· Dressing assistance as needed
The Pay Rate is $17 per hour.
Work Schedule--Up to 20 hours per week.
Willing to be flexible with a day shift or evening shift.
The ideal candidate for this opportunity will have some experience with special needs, be attentive and interactive, be able to take direction and follow cues, and be friendly. A driver's license and a reliable vehicle are required.

Community Horizons offers support for individuals enrolled in self-directed programs through Managed Care Organizations or Iris (Include, Respect, I Self-Direct) throughout Wisconsin. Self-directing gives program participants the choice over who provides services and how they receive them. Community Horizons is not the employer on record, members make all the hiring decisions with the involvement of a third-party fiscal agent responsible for payroll.
