The Pay Schedule - $16.32 per hour
Major Duties & Responsibilities
· Transfers using a Hoyer lift
· Showering assist-2 days per week
· Daily sponge bath
· Dressing assistance
· Medication Administration
· Empty Urinal
· Colostomy care
· Cooking
· Cleaning
· Laundry
· Errands-driving his wheelchair accessible van
Schedule
Hours needing a caregiver are Tuesday, Wednesday, and Thursday from 7:30 a.m. to 5:30 p.m. and fill-in/backup hours are available.
The ideal candidate for this opportunity will have a valid driver’s license, be reliable, friendly, and willing to learn.
Must be pet-friendly!
Community Horizons supports individuals enrolled in self-directed programs through Managed Care Organizations or IRIS (Include, Respect, I Self-Direct) throughout Wisconsin. Self-direction gives program participants the choice over who provides services and how they receive them.
