Wage $16.32 / Hour

Hours of Need

· Any needed pick-up hours as noted by the daily schedule below

· The member will attempt to give you as much advance notice to cover a shift as possible.

· The staffing schedule is completed by the end of each month for the entire upcoming month.

Regular Daily In-Home Schedule (for reference only)

· 8:30 am – 10:30 am

· 5:00 pm - 7:00 pm,

· 10:15 pm – 11:45 pm.

Tasks

* Assist with morning care, toileting, and dressing

* Assist with nighttime care, toileting, and undressing

* Toileting assistance (1-person pivot transfer)

* Light meal prep and general cleaning

Ideal Candidate

- Experienced and compassionate with personal care and transfers

- Familiar with the unique needs of individuals with cerebral palsy.

- Must have a reliable vehicle to get to work.

- Favorable Caregiver Background Check

- Pet Friendly (both dog and cat)

Community Horizons offers support for individuals enrolled in self-directed programs through Managed Care Organization or IRIS (Include, Respect, I Self-Direct) throughout Wisconsin. Self-direction gives program participants the choice over who provides services and how they receive them. Community Horizons is not the employer on record, members make all hiring decisions with the involvement of a third-party fiscal agent responsible for payroll.
