Position: In-Home Caregiver (PRN)
Location: Fon Du Lac, WI
Schedule: Late morning to early afternoon, averaging 2-2.5 hours per shift, as needed (PRN) M-W-TH 10am-12pm
Pay: $16.32 per hour

Community Horizons LLC. supports individuals enrolled in self-directed programs through Managed Care Organizations or IRIS (Include, Respect, I Self-Direct) across Wisconsin. Our participants have the freedom to choose who provides their services and how they receive them. All hiring decisions are made by the members.

Your Role:
- Assist with personal care tasks, including bedside baths and general hygiene.
- Perform light housekeeping duties to keep the home tidy.
- Prepare and serve nutritious meals.
- Support with dressing and undressing.
- Carry out range-of-motion (ROM) exercises to aid in mobility.
The Essentials – What We're Looking For:
- Must be at least 18 years old.
- Previous caregiving or healthcare experience is preferred
- Must be reliable and compassionate.
- A favorable background check is required.
- Is comfortable with personal care tasks, including bathing.
