Wage $16.32 / Hour
Hours of Need
· 10:15 pm – 11:45 pm
· 7 Nights per week are available
· Other pick-up shifts may also be available based on the daily schedule noted below
Tasks
- Assist with nighttime needs
- Toileting assistance (1-person pivot transfer)
- Assist with getting into bed
- Set out clothes for the next day
Ideal Candidate
- Experienced and compassionate with personal care and transfers
- Familiar with the unique needs of individuals with cerebral palsy.
- Must have a reliable vehicle to get to work.
- Favorable Caregiver Background Check
- Pet Friendly (both dog and cat)
Regular Daily In-Home Schedule (for reference only)
- 8:30 am – 10:30 am
- 5:00 pm - 7:00 pm,
- 10:15 pm – 11:45 pm.
Community Horizons offers support for individuals enrolled in self-directed programs through Managed Care Organization or IRIS (Include, Respect, I Self-Direct) throughout Wisconsin. Self-direction gives program participants the choice over who provides services and how they receive them. Community Horizons is not the employer on record, members make all hiring decisions with the involvement of a third-party fiscal agent responsible for payroll.
