Community Horizons is passionately supporting the search for a private caregiver to join a loving in-home care team. In this fulfilling role, you will provide vital support and hands-on assistance with personal care and housekeeping tasks. Your efforts will play a crucial role in helping this community member maintain their cherished independence at home.

This position is part of the self-direction program, which means you'll work directly for the individuals who need your support. This arrangement not only offers greater flexibility and control over your schedule but also fosters a more personal and meaningful working relationship, as the member will hire you directly, bypassing the agency.

Schedule:

- Mornings: 8:00 AM - 9:30 AM, Sunday through Saturday, with an additional 15 minutes on Mon, Wed, and Fri.

- Lunch: 12 pm – 1 pm, Sunday, Monday, Tuesday, Saturday

- Evenings: 7 PM - 8 PM, Sunday, Tuesday, Saturday

Pay:

- $20/hour (double time on holidays to show our appreciation for your hard work!)

Tasks:

- Hoyer lift: If you’re not sure how to use it, don’t stress! We’ll guide you!

- Helping with getting dressed and personal care to make their day better

- Enjoying nice, refreshing showers in the morning to feel great

- Cooking and sharing yummy meals together for some fun times

- Cleaning with care and a personal touch to keep things comfy

Sounds great! Here’s what you need to do well:

- You should be at least 18 years old.

- You need a driver’s license and a reliable way to get around!

- We really like people who care about helping others.

- Being friendly and dependable is super important!

- If you have experience as a CNA, that’s fantastic, but it’s not a big deal if you don’t.

- Just so you know, we’ll be doing a background check.

Community Horizons offers support for individuals enrolled in self-directed programs through Managed Care Organizations or IRIS (Include, Respect, I Self-Direct) throughout Wisconsin. Self-direction gives program participants the choice over who provides services and how they receive them. Community Horizons is not the employer on record; members make all hiring decisions with the involvement of a third-party fiscal agent responsible for payroll.
