Community Horizons is assisting with the recruitment process of amazing caregivers to work directly with a member in his home, allowing for flexible scheduling.
The schedule is part-time. Every other weekend for 48 hours and 1 day during the week for 8 hours on the opposite weeks.
Up to $17.12 per hour.
Tasks include but are not limited to:
Incontinence changing and toileting assistance.
Tube feeding (G-tube experience preferred).
Bathing
Dressing
Activities

Community Horizons offers support for individuals enrolled in self-directed programs through Managed Care Organizations or IRIS (Include, Respect, I Self-Direct) throughout Wisconsin. Self-direction gives program participants the choice over who provides services and how they receive them.
