Schedule: M, W, F (9 am-1pm)
Wage: $17 per hour (including mileage reimbursement)
Tasks include but are not limited to:
· Bathing Assistance and Skin Care
· Diabetes support
· Housekeeping
· Laundry
· Grocery Shopping
· Meal Prep
· Medical Appointment Support and Coordination
Ideal Candidate:
· Male
· Caregiving Experience
· Must be able to handle challenging behaviors
· Friendly
· Diabetic Knowledge
· Strong work ethic
· Good cook
Community Horizons offers support for individuals enrolled in self-directed programs through Managed Care Organizations or IRIS (Include, Respect, I Self-direct) throughout Wisconsin. Self-direction gives program participants the choice over who provides services and how they receive them. Community Horizons is not the employer on record, members make all hiring decisions with the involvement of a third-party fiscal agent responsible for payroll.
