Personal Care Companion

Wage: $16.32 – $17.00 / hour based on level of experience.

Hours

· Usual caregiving hours within the home are 8:00 am – 8:00 pm (12-hour shifts)

· Days and Hours of need vary at this time

· Pick-up shifts available

· Weekends and overnights as needed for vacation coverage

Tasks Include but are not limited to:

· Personal Cares

· Showering

· Repositioning, transfers, range of motion

· Other activities of daily living to include some skilled services

· Getting out into the community occasionally even if it is a stroll outdoors or visiting a local store

Ideal Candidate:

· Someone attentive, reliable, consistent, has a great sense of humor and can be interactive even though the person they are caring for is nonverbal.

· Has 1 year of professional experience in healthcare-related field

· Is familiar with the needs of someone with a brain injury and seizure disorder

· Familiar with or willingness to learn:

pump tube feeding, O2, nebulizer treatments, suctioning, ceiling lift, wheelchair safety, SPT Vest, and sponge baths. Training is available for the right applicant

· Has a reliable form of transportation to get to work

· Favorable Caregiver Background Check

Community Horizons offers support for individuals enrolled in self-directed programs through Managed Care Organization or IRIS (Include, Respect, I Self-Direct) throughout Wisconsin. Self-direction gives program participants the choice over who provides services and how they receive them. Community Horizons is not the employer on record, members make all hiring decisions with the involvement of a third-party fiscal agent responsible for payroll.
