What are the responsibilities and job description for the Personal Care Worker PM Shift position at Community Horizons LLC?
Community Horizons is thrilled to help a wonderful person in the Green Bay area find a new private caregiver to join her in-home care team! Looking for a rewarding position, in this role you’ll be assisting with transfers and provide support for personal care and light housekeeping around the house. Your kindness and support will truly make a difference in helping this community member feel comfortable and happy in her own home.
Available Hours: 5:30 pm - 7:30 pm daily. We will happily entertain any hours you can provide!
Starting Pay: $16.32 per hour.
Here’s what you can look forward to:
Hoyer Transfers
Assisting with personal hygiene needs.
Preparing some tasty meals.
Taking care of a little light housekeeping.
Helping reposition her in bed for added comfort.
We’re on the lookout for someone who:
Has a great sense of humor and enjoys chatting.
Is adaptable and goes with the flow.
Loves to cook.
Experience is a bonus, but it’s absolutely not a deal-breaker! We would love to chat!

Community Horizons provides support for individuals enrolled in self-directed programs through Managed Care Organizations or IRIS (Include, Respect, I Self-Direct) across Wisconsin. Self-direction allows participants to choose who provides their services and how they receive them. Please note that Community Horizons isn’t the employer on record; members will make all hiring decisions with help from a third-party fiscal agent responsible for payroll
