Community Horizons is passionately supporting the search for a private caregiver to join a loving in-home care team. In this fulfilling role, you will provide vital support and hands-on assistance with personal care and housekeeping tasks. Your efforts will play a crucial role in helping this community member maintain their cherished independence at home.

This position is part of the self-direction program, which means you'll work directly for the individuals who need your support. This arrangement not only offers greater flexibility and control over your schedule but also fosters a more personal and meaningful working relationship, as the member will hire you directly, bypassing the agency.

Schedule:

Position 1: Wednesday 4 – 8 pm, Saturday 9 am – 1 pm

Position 2: Back up Mon – Thur 4 – 8 pm, Saturday 9am – 1 pm

Pay: $16.32 / hour

Tasks:

- Tube Feedings – no experience needed, as we’re eager to provide training

- Offering gentle assistance with hygiene and dressing for a sense of comfort

- Utilize the Hoyer lift for safe and secure transfers

- Engage in uplifting entertainment – books, music, crafts, and more to bring joy to the day

What it takes to succeed:

- Must be 18

Community Horizons offers support for individuals enrolled in self-directed programs through Managed Care Organizations or IRIS (Include, Respect, I Self-Direct) throughout Wisconsin. Self-direction gives program participants the choice over who provides services and how they receive them. Community Horizons is not the employer on record; members make all hiring decisions with the involvement of a third-party fiscal agent responsible for payroll.
