Flexible Scheduling!
Schedule: Saturday and Sunday 3 shifts available 8a, 2p & 8p
Plus! Available PM hours during the week!
*Short Shifts-all shifts are 1 to 1.5 hours except for 2pm which is 2 hours*
Hiring multiple candidates!
Wage: $17 per hour
Tasks include but are not limited to:
· Meal Prep
· Housekeeping
· Med Support
· Personal Cares including bathing.
· Incontinence support and Toileting
· Transfers utilizing an Easy Stand
Ideal Candidate:

· Willing to take directions.
Community Horizons offers support for individuals enrolled in self-directed programs through Managed Care Organizations or IRIS (Include, Respect, I Self-direct) throughout Wisconsin. Self-direction gives program participants the choice over who provides services and how they receive them. Community Horizons is not the employer on record, members make all hiring decisions with the involvement of a third-party fiscal agent responsible for payroll.
