FULL TIME HOME HEALTH AIDE POSITION
6:30AM to 3PM with OCCASIONAL WEEKENDS AND HOLIDAYS
SUMMARY: As a member of the multidisciplinary team, the Hospice Aide (CHHA) works under the direction of the Patient Care Manager and the supervision of the registered nurse/case manager in the provision of personal care, in order to maximize the comfort and quality of life of patients and families. Responsible for providing supportive care in accordance with the plan of care and Community Hospice policies and procedures.
RESPONSIBILITIES:
- Assists with environmental services such as housekeeping and homemaking services to preserve a safe, sanitary home and to enhance quality of life in accordance with the CHHA assignment and the hospice plan of care.
Standard: Home visits and activities are completed according to the hospice aide assignment and the plan of care.
2. Assists the patient with personal care and supportive services in accordance with the CHHA assignment and the hospice plan of care.
Standard: Personal care will be provided exactly as planned and assigned (both visit frequency and tasks) by the registered nurse.
3. Communicate with other hospice team members as needed to report patient changes and needs and requested changes in the care assignment related to patient changes.
Standard: Communications with other team members will be done on the day of patient contact.
4. Submits visit notes and timesheets in a timely manner.
Standard: Submits visit notes and timesheets within 24 hours of visit.
5. Cooperates with regulatory requirements for supervisory visits by the case manager.
Standard: Schedule visit appointments and reliably adhere to schedule so that supervisory visits can be completed in a timely manner.
6. Communicates expected arrival times to patient and family on scheduled day.
Standards: All patients/families will be called the afternoon prior or the AM of a scheduled visit to inform of the expected arrival time within 1/2 hour. The patient/family will be notified if the CHHA will be more than 15 minutes later than expected.
7. Attends regularly scheduled care conferences (IDT) to review cases, problem-solve and cover unique issues, share professional support and exchange feedback aimed toward enhancing professional growth.
Standard: Attendance at weekly conference encouraged, unless assigned essential patient care by the Patient Care Manager.
8. Attends death events and provide appropriate support at time of death, as requested.
Standard: Attends death events as requested.
9. Participation in Community Hospice orientation of students.
Standard: Make joint visits with students, as assigned, and assists in the teaching of hospice concepts.
10. Participates in inservices, workshops, and clinical meetings to promote professional growth and understanding of hospice care.
Standard: Attendance at 80% of all mandatory scheduled inservices and meetings, designated as CHHA appropriate, provided by agency.
11. Follows all Community Hospice policies and procedures including, but not limited to, safety, infection control, privacy, confidentiality, and HIPAA.
Standard: 100% compliance
12. Maintains a professional appearance of self and working area; exhibits appropriate behavior with guests and clients, i.e., courteous, thoughtful and professional at all times.

Qualifications:
- Experience in providing homemaking and personal care services in the home setting highly desirable.
- Must have worked in the past 24 consecutive months for compensation.
- Ability to read and write.
- Possesses and maintains current CPR certification.
- Valid California Home Health Aide Certificate must be maintained at all times.
- Valid Driver’s License must be maintained at all times.
- Valid California Auto Insurance must be maintained at all times.
WORK ENVIRONMENT: The main environment for this position is in the patients' homes, which vary in levels of cleanliness, accessibility, and climate control. The employee may be exposed to virus, disease and infection from patients. The employee may experience situations, with emotionally disturbed, substance abuse, serious illness, and deceased patients. While performing the duties of this position, the employee will travel by automobile and is exposed to changing weather conditions.
WORK SCHEDULE: Position is regular full-time required to work a regularly scheduled 40-hour workweek, with overtime required to meet patient needs. Position may also require rotating weekends and holidays, positions may be occasionally utilized, if justified by department needs.
