What are the responsibilities and job description for the Hospice Quality Improvement Coordinator - In Foley Office position at Community Hospice of Northeast Florida?
The QUALITY ASSESSMENT PERFORMANCE IMPROVEMENT COORDINATOR is responsible for the planning, gathering, monitoring, analyzing and reporting of all data that represents the agency's performance improvement and patient safety monitors and outcomes for regulatory and internal purposes. This position will also assist with orientation and continuing education of clinicians.
This position requires working from the Foley office, and is not remote.
Requirements
RESPONSIBILITIES :
- Assists in the development of all QAPI activities and education with appropriate data collection, aggregation, analysis, taking action and reporting of results according to agency's QAPI plan
- Reviews patient medical records for compliance with federal, state and local laws, accreditation standards and agency policies and guidelines
- Chairs QAPI committee, prepares QAPI reports / minutes of meetings, forwards reports according to QAPI plan
- Responsible for weekly and monthly chart compliance audits
- Understands and promotes principles of continuous QAPI
- Responsible for the orientation of new staff to agency's QAPI program
- Assists in the training and education needs of staff
- Assists in the preparation and implementation of policies and procedures which meet Medicare and Medicaid accreditation standards, and state and local laws
- Participates in studies and other administrative functions assigned
- Serves as a resource to employees
- Investigates and reports any problem related to patient care and / or employee well-being
- Immediately reports any incident, accident, lost articles or unusual occurrence
- Attends pertinent continuing education programs other than routine in-services and shares information with staff
- Performs other duties as assigned by Executive Director
