PACE Partners of Northeast Florida, also known as PACE (Program for All-Inclusive Care for the Elderly), is a joint venture between Community Hospice & Palliative Care and Aging True. Care coordination for the program, the first of its kind in North Florida, is delivered at our beautiful day center known as The PACE Place.

Our clinic and adult day center, located on the Westside of Jacksonville, is next to the Acosta-Rua Center for Caring. Under the direction and supervision of the Homecare Coordinator, the Certified Nursing Assistant (CNA) provides personal care services to PACE participants and performs related tasks. The CNA is responsible for performing duties assigned in order to increase the level of comfort, maintain personal hygiene and ensure a safe environment for the participant. As scheduled by the Home Care Coordinator, the CNA will work at the adult day center and in participant homes to provide services identified in participant care plans. This includes ensuring day center activities are organized and encouraging participation as well as assisting the participants in bathing, laundry, other daily living tasks, and escort services to medical appointments. The CNA is a member of the PACE Interdisciplinary Team (IDT).
Qualifications and Requirements :
- High School Diploma or GED, preferred
- Current CNA certification in Florida
- Level II AHCA Background Check
- Valid Florida driver's license
- Active Automobile Insurance Coverage
Experience
Minimum of 1-year CNA experience working with the frail or elderly population preferredSkills and Knowledge

Assist participants with activities of daily living, provide personal care for participants, and provide light housekeeping duties as needed and directed in the PACE Center or Home Care team and the participant's plan of careProvides emotional support to participants and their familiesObserve for physical and emotional changes and report to the Home Care Coordinator or Center ManagerSurveys and observes for safety issues and reports to Home Care Coordinator or Center ManagerMay take, record pulse, respiration, and temperatureMaintains daily cleanliness of participant restrooms, the day center, and ensures participants are assisted in restroom activitiesMay provide laundry and bathing duties as requestedParticipates and guides participants in day center activities and meal and snack serviceReports all complaints and grievances made by the participant to the Home Care CoordinatorTreat all participants fairly, with kindness, dignity, and respectRelay patient-related information efficiently and effectively to appropriate staff membersMaintains up to date records using the EMR system so problems, plans, actions, and goals are accurately and clearly stated according to PACE Place policies, best practices, and regulatory requirementsSuccessfully completes the annually required compliance and in-service trainingParticipates in QAPI program as appropriateAttends regular staff meetings as scheduledDisplays critical thinking skills to problem solve and manage patient and family issues according to PACE Center or Home Care team, and the PACE Place policies and proceduresInforms the Home Care Coordinator or Center Manager of unusual, problematic patient / family issuesParticipates in emergency disaster processes as defined by PACE PLACE policiesDemonstrates familiarity with PACE Place policies / procedures and follows state and federal regulatory requirementsParticipates in Inter-Disciplinary Team (IDT) meeting, as able and availableMaintains all HIPPA and confidentiality of participantsFollows all PACE Place Policies, Procedures, and OSHA safety guidelinesMaintain the confidentiality of all company procedures, results, and information about participants, clients, or familiesManage potentially infectious specimens with appropriate biohazard precautions and practices standard precautionsMaintain a safe working environmentPerforms any other duty assigned by the Home Care Coordinator or Center ManagerWe are an equal opportunity employer.
