This position is responsible for providing spiritual/pastoral services and counseling to patients and families/caregivers, in accordance with an established plan of care and acts as a liaison between patients/families and their identified faith community. The staff chaplain provides spiritual/pastoral counseling to Community Hospice and Palliative Care staff and volunteers as identified and appropriate.
- Provides spiritual/pastoral assessment within organization established timeframes and as indicated and appropriate.
- Provides spiritual/pastoral support in accordance with on-going wishes and needs of the patient/family including religious services, prayers, worship and/or rituals for patients/caregiver as indicated in the plan of care and in keeping with their beliefs.
- Develops individualized spiritual/pastoral plans of care which:
- Demonstrates an effort to work in close collaboration with local clergy whenever desired by the patient; and
- Provides spiritual support as needed and defined by the patient/family/caregiver.
- Conducts religious services of prayer, worship and rituals for patient/families, as appropriate.
- Offers patient/families/caregivers of different philosophies and religious beliefs opportunities to discuss and share their thoughts, feelings, beliefs and values.
- Documents counseling and all patient/caregiver interactions provided in the patient’s electronic medical record (EMR) according to Community Hospice and Palliative Care policy.
- Works with other professionals in resolving spiritual/pastoral/ethical issues.
- Participates as an active member of the Interdisciplinary Group (IDG) and assists in the development and updating of the plan of care.
- Assesses patient/family/caregiver response to and satisfaction with care.
- Participates in development of approaches to meet staff counseling needs and is available to staff for bereavement support.
- Meets with individual staff regarding personal spiritual/pastoral issues that may affect their ability to function effectively.
- Assists in the training, mentoring, utilization, and supervision of select volunteers as an extension of spiritual care services to Community Hospice and Palliative Care patients and caregivers.
- Serves as an advocate for patients and caregivers related to the expression of spiritual needs.
- Completes all department reporting requirements within the assigned timeframes.
- Assists the patient caregiver with planning and/or conducting funerals or memorial services as requested and/or assigned.
- Provides education on spiritual and bereavement topics to staff, volunteers and other interested individuals/organizations in the community.
- Participates in organization and departmental meetings; in-service training sessions and contribute to team goals.
- Complies with professional and ethical standards in delivering services to patients and caregivers.
- Plans and coordinates Services of Remembrance/memorial services for Community Hospice and Palliative Care families, long term care facilities, assisted living facilities and the community at large as assigned.
- Networks regularly with other faith group clergy, hospital chaplains, and seminary clergy, in order to better utilize resources and increase the visibility of the Community Hospice and Palliative Care program.
- Participates in the regular on-call rotation of the department to ensure the availability of spiritual care services on a 24/7 basis.
- Other duties as assigned by the Director of Spiritual Care.
Education & Credentials:

Master of Divinity with at least 72 credit hours from a seminary accredited by the Council for Higher Education Accreditation (www.chea.org).

Completion of a minimum of four (4) units of ACPE Clinical Pastoral Education (www.acpe.edu) , preferably including one to two CPE units completed as a fulltime chaplain resident.
- Board Certified Chaplain (or eligible) with the Association of Professional Chaplains or the National Association of Catholic Chaplains, or the National Association of Jewish Chaplains, or the Canadian Association for Spiritual Care.
- Ecclesiastical Ordination or the Equivalency Thereof by the Individual’s Faith/Belief System Affiliation.
- Official, Specific Endorsement by the Credentialing Body for the Office of Chaplaincy Ministry.
- Current State of Florida driver’s license with driving record acceptable to Community Hospice and Palliative Care insurance carrier, and reliable transportation.
Experience:
Minimum of five (5) years’ experience in pastoral care, with at least one (1) year of which was in a health care setting and preferably including experience in a hospice or palliative care setting.
Knowledge of:
- The hospice philosophy and end-of-life care.
- Standards of practice and ethical standards for practice of the pastoral care professional.
- Various religious/spiritual beliefs, traditions and practices, particularly as they pertain to the delivery of end -of-life care.
