CHMHA has a fantastic opening for a Housing Support Specialist.
Our Housing Support Specialist will work in a team approach to provide wrap around support services to adults with severe and persistent mental illness, assisting them to successfully live independently and integrate into the greater community.

SCHEDULE: Day Shift, Tuesday-Saturday (35 hours-Full Time)
RESPONSIBILITIES
- Provide assistance in accordance to individual treatment plans such as hygiene needs, meal preparation, daily skills groups, and community outings
- Assess living spaces for cleaning purposes on a weekly basis
- Coordinate client needs such as appointments, engagement with community support networks, vocational services, or other applicable needs
- Provide immediate crisis intervention and resolution as needed
- Review daily log and documents after each shift and communicate other staff members of any emergency situations
- Visually supervise medication distribution as necessary
- Lead assigned life group skills
- Maintain client records in keeping with agency, county, state, and federal guidelines.
- Provide input to primary psychiatric provider and/or case manager on client progress and treatment plan
- Facilitate assigned group activity sessions and specialized client events such as field trips, holiday parties, etc
- Assist in admission and discharge planning to community based supported housing programs when directed
- Coordinate client orientation and monitor/inform property manager of room maintenance as needed
- Attend scheduled staff meetings and community meetings when assigned.
- Commit to minimum 10 hours of annual continuing education/training
- Maintain client and staff confidentiality at all times
BASIC QUALIFICATIONS
- High School Diploma/GED with minimum 2 years experience
- Current First Aid and CPR certification
- Current Food Handler permit
- Have legible handwriting
- Ability to do simple mathematics
- Ability to read and comprehend agency/client charts
- Valid driver’s license and clean driving record for use of agency vehicles
- Experience with target population or ability to acquire specialized training to work with target population.
- Ability to work independently and in a team.
- Be free of substance use
- Ability to perform duties outlined in description within reasonable accommodation
PREFERRED QUALIFICATIONS
- Bachelor’s degree in social work or other related field
- Working knowledge and adherence to all agency policies and procedures
