Job Description

- Provide psycho-social assessments and interventions for Community LIFE participants and their families. ">
- Perform utilization case management for all acute inpatient admissions and transfers. ">
- Maintain a caseload of community-based participants and provide support services as needed. ">
- Collaborate with healthcare teams to develop and implement comprehensive care plans. ">
Required Skills and Qualifications
">We are seeking a skilled and compassionate individual with:

- A Master's Degree in Social Work (MSW) from an accredited school of social work. ">
- A minimum of one year experience working with the frail or elderly population. ">
- Geriatric social work experience in a hospital, long-term care, or community setting. ">
- Excellent verbal and written communication skills. ">
- The ability to work effectively with people from various socio-cultural backgrounds. ">
- The ability to accept and utilize supervision. ">
- The ability to work independently and be self-directed. ">
- Good problem-solving skills. ">
- A compassionate and empathetic attitude towards the frail older adults. ">
- A valid PA Driver's license. ">
