- Providing psycho-social assessments and interventions related to community maintenance.
- Performing utilization case management for all acute inpatient admissions and transfers.
- Collaborating with healthcare professionals to develop comprehensive care plans.
Requirements
- Master's Degree in Social Work (MSW) from an accredited school of social work.
- Minimum one year experience working with the frail or elderly population.
- Excellent communication skills, with the ability to work effectively with people from diverse backgrounds.
- Able to work independently and be self-directed, with good problem-solving skills and a compassionate attitude towards older adults.
