What are the responsibilities and job description for the Geriatric Care Manager position at Community LIFE?
Key Responsibilities
- Assess participant needs and coordinate delivery of participant-centered appropriate home/personal care services
- Work closely with our interdisciplinary team to develop and implement effective care plans
- Coordinate care services with participants, providers, and referral sources
- Participate in weekend and on-call rotation
- Collaborate with our Therapy team to maximize rehab potential and minimize inpatient care
