Key Responsibilities

  • Assess participant needs and coordinate delivery of participant-centered appropriate home/personal care services
  • Work closely with our interdisciplinary team to develop and implement effective care plans
  • Coordinate care services with participants, providers, and referral sources
  • Participate in weekend and on-call rotation
  • Collaborate with our Therapy team to maximize rehab potential and minimize inpatient care
