What are the responsibilities and job description for the Health Physical Therapist position at Community LIFE?
Shift : Mon-Fri, 7 : 30am-4 : 00pm
Location : Bedford, PA
Summary : Community LIFE is an all-inclusive program that empowers older adults (age 55 ) to remain at home, while preserving their dignity, independence, and quality of life. Our Physical Therapist (PT) works with Community LIFE participants in an effort to regain or compensate for mobility, strength, range of motion, pain, and / or discomfort with the expected outcome of helping improve their quality of life and independence. The PT also performs utilization case management for all inpatient admissions and transfers to Skilled Nursing Facilities. The PT directly supervises the PTA. The PT will be required to make in-home visits as necessary for assessment and on-going case management. As part of an interdisciplinary care team, the PT will collaborate with physicians, nurses, and other therapists to develop and implement individualized care plans.
Required Education & Experience :
Must be a graduate of a physical therapy curriculum approved by the American Physical Therapy Association (APTA) with the minimum of a Bachelors degree and at least 2 years experience with the geriatric population.
Required Licensure / Certifications
- Valid PT license in the state of Pennsylvania.
- Valid Drivers license.
