Job Summary: We are seeking a skilled Registered Nurse to join our team as a Home Health Visiting RN Case Manager. In this role, you will provide in-home assessments and overall care-management to a caseload of frail older adults. The ideal candidate will have a background in home care or hospice and a passion for managing geriatric care-planning in a team environment.
The RN Case Manager will be responsible for assessing the need for, and coordinating the delivery of, nursing and home care services. This position involves local travel outside of the office, seeing patients for in-home assessments. The remainder of the day is spent visiting patients in our day center medical clinic, attending interdisciplinary meetings, intake & assessment care-planning meetings, and office time for documentation.
The successful candidate will be a Graduate of an accredited nursing school with at least one year of experience working with a frail elderly population, preferably in long-term care, home health care or community health setting. Experience teaching patients and other healthcare workers preferred.
